Overview

The Buffalo Federation of Neighborhood Centers (BFNC) was awarded $500,000 to provide direct, one-on-one intensive benefits access and case management services.


Projects

East Buffalo Engagement Project - $500,000

The Buffalo Federation of Neighborhood Centers Inc. (BFNC) is provided ‘door to door’ canvassing of households in East Buffalo, with a focus on the 14204, 14208, 14209, 14211, 14212 and 14215 ZIP codes to provide information about available benefits and services. In addition, the BFNC outreach team participated in a variety of public events each month to connect with residents within their communities.

Using culturally competent, trauma informed methods, BFNC provided, specialized case management specialists who can identify specific social services, housing, and similar needs, take immediate action to address these needs, and make referrals (and track progress) of benefits cases. When possible, these trained specialists conduct door-to-door outreach to ensure residents have direct access to assistance. Examples of benefits and services that the team provided direct access to include, but are not limited to:

  • Public Assistance
  • Supplemental Nutrition Assistance Program
  • Utility assistance including the Home Energy Assistance Program (HEAP)
  • Mental health and behavioral health services
  • Referrals for care management
  • Access to socialization activities
  • Rental arrears assistance and/or mortgage assistance
  • Health insurance coverage, including assistance through Medicaid, Child Health Plus and other programs
  • Property tax relief through STAR
  • Assistance with housing code compliance
